These Terms and Conditions apply to the promotional offers listed below. By accessing or using these offers, patients acknowledge and agree to the conditions set out here.
1. Eligibility
- Offers are available to new patients of On Point Dental, unless otherwise specified.
- A new patient is defined as someone who has not attended On Point Dental in the past 24 months.
- Offers are non-transferable and limited to one use per patient.
2. Availability
- Offers are subject to appointment availability.
- Offer is valid until withdrawn by On Point Dental.
- On Point Dental reserves the right to withdraw or modify offers at any time without prior notice.
3. Offers Included
a. New Patient Offer — Complete Preventive Care Package – $299
- Available to new patients only.
- Package includes: scale and clean, fluoride treatment, and necessary X-rays in one appointment.
- The specific X-rays provided will be determined based on clinical assessment and may include diagnostic imaging as required for a comprehensive oral health evaluation.
- Price of $299 applies to the complete package as described.
- Does not include additional treatments, procedures, or follow-up appointments beyond the services listed.
- Health fund benefits may be claimable depending on your level of cover. Patients should confirm their benefits with their health fund prior to the appointment.
4. Terms of Use
- Offers cannot be used in conjunction with any other promotion, discount, or payment plan.
- Non-redeemable for cash or any other service.
5. Health Fund Clarification
- Patients with private health funds may be able to claim benefits for services included in this package, subject to their level of cover and annual limits.
- Patients are encouraged to confirm their level of cover with their health fund before attending.
- On Point Dental is not responsible for out-of-pocket costs where fund limits are exceeded or where services are not covered by the patient’s policy.
6. Scope of Offers
- Offers are limited to the services specifically described.
- They do not include follow-up appointments, additional treatments, or other dental services unless expressly stated.
- Any additional treatment recommended following the initial appointment will be subject to standard fees.
